AN INVITATION TO PREQUALIFY/APPLY FOR ELIGIBILITY AND TO BID

The Cebu Port Authority, North Reclamation Area, Cebu City, invites accredited and duly registered contractors to participate in the public bidding of the following projects :

1. Name of Project: REPAIR OF WEARING COARSE @ WORKING APRON FROM BERTHS 2 - 6, CIP COMPLEX, PORT OF CEBU

A. Scope of Work :

Supply / Spreading in place of Bituminous Prime Coat and Bituminous Concrete Surface Course

B. Classification : Small B for Roads

C. Equipment Requirements :

2. Name of Project: REPAIR OF PAVEMENT @ CY BLOCK A INCLUDING ROCK LANE, CIP COMPLEX, PORT OF CEBU

A. Scope of Work :

a) Removal of Damaged Concrete Paving Blocks, Stripping / Excavation of Excess Base Coarse Materials and Disposal (A=10,212 sq.m.)

b) Grading and Compaction of Existing Base (A=10,212 sq.m.)

c) Pouring of 0.30m thk. (3,500psi @ 14 days) Concrete; V = 3,064 cu.m.

d) Concrete Cutting, Curing and Sealing (A=10,212 sq.m.) including Stripping of Forms

e) Declogging of Existing Drainage System

f) Repair of Damaged RTG Beam

g) Safety, Health and Security

1 unit Bulldozer (D6 140 fly Hp) - Owned/Leased

1 unit Water Truck - Owned/Leased

1 unit Road Roller (Vibro 10 m.t. cap) - Owned/Leased

1 unit Road Grader (125 Hp) - Owned/Leased

2 units Concrete Screeder - Owned/Leased

Prospective bidders shall submit to the BAC Chairman of this office their duly accomplished Prequalification or Eligibility Statements in HARD/BOOK-BOUND form. Prequalification or Eligibility Statements submitted other than Hard/Book-Bound form shall not be accepted.

Bid securities in the form of surety bond, Callable on Demand, can be secured from the GSIS or other private insurance & surety companies duly accredited by the office of the Insurance Commission.

Prospective bidders shall submit the necessary documents or evidence to prove ownership of equipment and the duly notarized lease contract indicating the brand, serial number, capacity, and present location of equipment being leased.

Prospective bidders are required to present the original copy of Contractor's License/Certificate of Renewal and IAC Classification and Registration for verification/authentication before Pre-qualification or Eligibility Statement shall be issued.

All particulars relative to prequalification, bidding, evaluation of bids, and awards of contracts shall be governed by the provisions of R.A 9184, and other rules and regulations as may be applicable.

CPA reserves the right to reject any or all bids, to waive any formality/defect therein, and to accept such bid as may be deemed most advantageous to the government.
